You will need to complete the following forms and provide the following information to process your financial aid application at Ohio Technical College.
If you DO NOT plan to apply for financial aid, please complete the FA Refusal Form.
Review the following checklist to determine your dependency status. If your answer is “yes” to any of the following 8 questions, you are considered an independent student for financial aid purposes (additional documentation may be required to support your dependency status.)
Dependent or Independent Checklist
1. Were you born before January 1, 1986?
2. As of today, are you married? (Answer "yes" if you are separated but not divorced.)
3. Do you have children who receive more than half of their support from you?
4. Do you have dependents (other than your children or spouse) who live with you and who receive more than half of their support from you, now and through June 30, 2009?
5. Are (a) both of your parents deceased, or (b) are you (or were you until age 18) a ward/dependent of the court?
6. Are you a veteran of the U.S. Armed Forces? Answer “yes” if you have engaged in active duty in the U.S. Armed Forces (Army, Navy, Air Force, Marines or Coast Guard) or are a National Guard or Reserve enlistee who was called to active duty for purposes other than training and were released under a condition other than dishonorable. Also answer “yes” if you are not a veteran now but will be one by June 30, 2009.
7. Are you or were you an emancipated minor, or are you or were you in legal guardianship as determined by a court in your state of legal residence?
8. At any time on or after July 1, 2008, did your high school homeless liaison or director of emergency shelter, transitional housing or youth basic center determine that you were an unaccompanied youth who was homeless or self-supporting and at risk of being homeless?
1. Complete the Institutional Verification Form (IVF).
2. Send a legible copy of the STUDENT’S and PARENTS’/SPOUSE'S 2008 W-2 forms.
3. Send a legible copy of the STUDENT’S and PARENTS’/SPOUSE'S signed 2008 Federal Tax Return along with attached schedules.
4. Send a legible copy of 2008 Social Security benefits received, if applicable.
5. Send a legible copy of 2008 non-taxable income your family received, such as disability benefits, child support, pensions, public assistance, or Workman’s Compensation, if applicable.
6. Complete the Free Application for Federal Student Aid (FAFSA) at www.fafsa.ed.gov. (OTC's school code is 011745.) You will need your FAFSA PIN to electronically sign the form. If you do not have a PIN, you may apply for one at www.pin.ed.gov.
7. Send a copy of the student's FINAL high school transcript, GED certificate, or ATB results (if not previously submitted). If you need to request this information from your high school, please use the Request for High School Transcript.
8. DEPENDENT STUDENTS ONLY - Complete the Federal Direct Parent Loan for Undergraduate Students (PLUS) Pre-Application Form.
